Congressional Recognition

If you would like to request a Congressional Recognition for someone living in the 8th Congressional District, please complete and submit the form below.  You can also print the form and mail it to my Arizona office. The types of recognitions are listed below. Be sure to select the proper request in the field near the end of the form.

Veteran Recognition: Presented to an individual who has served or is serving our Country and is giving back to their community in a volunteer capacity. This recognition is to honor Veterans of the AZ-8 community who strive to make the lives of others better. 

Student RecognitionPresented to a student in AZ-8 who has achieved an educational milestone and/or exceeded expectation for their own learning, exhibited extraordinary self-sacrifice, or who have gone above and beyond to serve/aid the school, community, state, and/or country. This recognition is to honor students who strive to make the lives of others better. 

First Responder RecognitionPresented to an individual who has protected the citizens in our community and sacrificed their own safety for our security. This recognition is to honor First Responders of the AZ-8 community who strive to make the lives of others better. 

Community Advocate Recognition: Presented to an individual or non-profit, church or charity in AZ-8 who has assisted citizens in their community. This recognition is to honor members of the AZ-8 community who strive to make the lives of others better.
